Please distinguish between fraudulent documents used for provider enrollment, claims, billing, time sheets and medical records and fraudulent enrollment documents. Don't listen to the rants of right wing media. Electronic verification of citizenship, Social Security numbers, and immigration status has been mandatory for Medicaid and CHIP since the ACA rolled out in 2014. An applicant may be asked for or volntarrily provide paper proof, but electronic verification is required. Electronic verification is Required for SSN, citizenship and immigration status. States only collect paper proof of income and assets if electronic verification is not available. Many working Medicaid and CHIP folks aren't required to file income tax returns and others dont have their income show up in quarterly wage dat or TWN. Most folks do have income verified as well.

Eligibility fraud is increasingly rare. Provider fraud is common.
